SCHOLARSHIP GUIDELINES & APPLICATION

Scholarship Award:

  • Three scholarships will be awarded annually: (1) $5,000, (1) $3,000, and  (1) $2,000. (The number and amount of the scholarships awarded will be dependent on annual funding available.)
  • Any scholarships awarded are to be used for attending a college, university, or trade school located in the United States while having Cystic Fibrosis (CF).
  • The funds are to be used for tuition, fees, books, school supplies, meal plans, or housing costs.
  • No award will be paid to the recipient - all awards will be paid directly to the school of choice.
  • There is no limit to the number of years a person may apply for this scholarship, but an application must be submitted each year to be considered.
  • An individual can only be awarded a maximum of four scholarships.
  • No special consideration will be given to previous recipients.

Eligibility Criteria:

Applicants must meet the following criteria:

  • Applicants must have Cystic Fibrosis. A confirmation of your CF diagnosis must be provided on the medical center’s letterhead stationery and signed by a licensed physician, nurse practitioner, physician assistant, or certified practitioner directly involved in your care. The letter must include the provider’s name, title, and contact information, confirmation of your CF diagnosis, and a statement verifying their role in your treatment. (The "Healthcare Team Member Letter of Recommendation" listed below may be combined with this letter.)
  • Applicants must be a citizen of the United States. Examples of acceptable citizenship documentation include an official birth certificate issued by a U.S. state, a U.S. government-issued certified birth certificate, a valid, unexpired U.S. passport, or a U.S. certificate of birth abroad.  (We no longer accept a driver's license as proof of citizenship.)
  • Applicants must be a resident of the Central United States.You must reside in one of the following states: Alabama, Arizona, Arkansas, Colorado, Illinois, Indiana, Iowa, Kansas, Kentucky, Louisiana, Michigan, Minnesota, Mississippi, Missouri, Montana, Nebraska, New Mexico, North Dakota, Ohio, Oklahoma, South Dakota, Tennessee, Texas, Utah, West Virginia, Wisconsin, or Wyoming.
  • Applicants must have a cumulative GPA of 3.00 on a 4.00 scale. You must be in good academic standing, and not currently on academic warning or probation.
  • Applicants must be enrolled in, or accepted to, an accredited junior college, college, university, or trade school located in the United States. You must submit proof of admission, acceptance, or enrollment.  Incoming college freshmen must supply an official high school transcript and a copy of their acceptance letter or enrollment confirmation from their college, university, or trade school. Current college or university students must supply an official transcript and enrollment confirmation from their college, university, or trade school.
  • Applicants must submit a recent photo, in JPG or PNG format, of themselves for identification and, if receiving an award, use on our website.

Letters of Recommendation:

Applicant must provide THREE letters of recommendation:

  • One from a Healthcare Team Member. Any physician, nurse practitioner, physician assistant, or any certified practitioner directly involved in treating your CF. The confirmation of your CF diagnosis may also be included in this letter if desired.
  • One from a School Official. Any employee of the school in which you are enrolled that you have involvement with during school year. This includes, but is not limited to, teachers, counselors, or coaches.
  • One from a Community Member. Someone whom you know well - such as an athletic instructor, coach, pastor, or any other person you know from your community involvement.
  • The letters MUST include the contact information of the writer for possible follow-up and verification.

In their letters, they should let the Selection Committee know:

  • What personal qualities, examples of achievements, or scenarios that support their endorsement?
  • What would make you a good candidate for this scholarship?  They should base it on the time they have spent with you through whatever relationship they have had with you.

Applicant Essay:

Applicants are to write an essay telling us about yourself. For example:

  • What are your future goals and aspirations?
  • What are your educational goals?
  • How has your medical condition and treatments affected your life?  
  • How you have accomplished something despite your Cystic Fibrosis?  Something where CF should have or could have been the obstacle to your success.
  • How continuing your education will benefit your future?
  • Please provide any additional information you may want the committee to consider in evaluating your application.

The essay helps the Selection Committee learn more about the applicant in a much broader and more informal way.

Selection Criteria:

  • Scholarship winners are determined solely by the Selection Committee and all decisions of the Selection Committee are final.
  • In selecting the applicant, the selection committee will take into consideration each applicant’s scholastic record, school activities, leadership, and community service.
  • Awards are primarily merit-based, but financial needs will be considered as well.

Submission Instructions:

Please read these instructions carefully.  If you have any questions, please e-mail the Scholarship Administrator at: admin@jmebax.org.

  • The application and all related documents, must be included for the application to be reviewed.  If there are items that are not relevant to you, please write N/A.
  • Please verify that ALL sections of the application are filled out.
  • Please redact any social security numbers from your documents before sending them.
  • Incomplete, inaccurate, or misleading information or late applications will NOT be considered.
  • Your parent or legal guardian must sign if you are under 18 years of age.
  • Please verify that you have submitted ALL of the necessary documentation when submitting your application. (Download and review the scholarship checklist linked at the bottom of this page.)
  • All materials must be sent in .PDF format only - NO MS Word docs, Google docs, etc. and NO photos of the documents. (The only exception to this, is the "Proof of U.S. Citizenship" document may be provided in either a .pdf or .jpg format.)
  • The photo submitted must be sent in a .JPG or .PNG format only - NO .PDFs. (Do not send photos that have a photographer’s watermark on them. These are copyrighted by the photographer and cannot be used.)
  • Please verify that all of the documents are clear and legible before submitting them.
  • All documents submitted are to be sent as individual PDF documents and not one large file.
  • Please submit your completed application, along with all required documents, to the Scholarship Administrator at admin@JmeBax.org.
  • A confirmation email will be sent after the receipt of your completed application.

We are not responsible for any technical issues that prevent applications from being submitted or considered.

Important Dates:

  • The application packet will not be accepted before Jan. 1st and must be received on or before  May 1st.
  • All applicants will be notified by June 15th if they were recipients or not of a scholarship award.
